Background
The proportion of elderly people (65 or over) in Japan was 26.2% in 2015, the highest in the world, and is predicted to be 39.9% by 2060. Home care has been promoted as a response to this aging population. Many such patients need guidance on the many drugs they are prescribed, so the role of pharmacists is very important.
