Background
Clinical pharmacy services at an Australian metropolitan hospital network are undergoing a redesign to be able to provide services through a seven day, activity based model. As part of redesign, extensive literature review was conducted to develop criteria to identify patients at greatest need for pharmacist review. The criteria focused..
