Background
About 30% of all hospital discharges are delayed for non-medical reasons including poor organization of post-discharge health, poor knowledge of social circumstances, and poor communication between the hospital and community pharmacy.Purpose
The objective of this study was to evaluate the outcomes of a pilot Re-engineered Pharmacy.. The access to the whole abstract and if available the presentation file is available to FIP members and to congress participants of that specific congress.
